Medical (military medical) commission

MEDICAL (MILITARY-MEDICAL) COMMISSION

The Medical (Military Medical) Commission was established to conduct a medical and military medical examination to determine the suitability of candidates for service in the National Police and the National Guard, candidates for admission to higher educational institutions of the Ministry of Internal Affairs, the suitability of police officers and servicemen for their transfer and dismissal.

LIST OF MEDICAL DOCUMENTS FOR PERSONS ENTERING THE SERVICE OF THE NATIONAL

POLICE OF UKRAINE

- REFERRAL FROM THE PERSONNEL DEPARTMENT FOR MEDICAL EXAMINATION IN THE FORM (ANNEX 4).

- MILITARY ID OR MILITARY REGISTRATION CERTIFICATE FOR MEN, PASSPORT FOR WOMEN.

- OUTPATIENT CARD FROM THE PLACE OF RESIDENCE.

- DETAILED EXTRACT FROM THE MEDICAL RECORD OF THE MEDICAL AND PREVENTIVE

- INSTITUTIONS AT THE PLACE OF RESIDENCE, WORK, SERVICE, OR STUDY FOR THE LAST 5 YEARS (the extract must be certified by the doctor's signature and the seal of the medical institution).

- CERTIFICATE OR REFERENCE OF PSYCHIATRIST AND NARCOLOGIST (at the place of registration).

- CERTIFICATE OF MANDATORY PROPHylactic vaccinations (certified by the seal of the institution).

- ZAG. BLOOD TEST, URINE TEST, AN. BLOOD GLUCOSE TEST, AN. BLOOD FOR RV, HEPATITIS B, C; HIV, BLOOD BIOCHEMICAL ANALYSIS (bilirubin, ALT, AST, thymol test, cholesterol, urea, creatinine), BLOOD GROUP AND RESULT OF ABSTINENCE.

- X-RAY (FLUOROGRAPHIC) EXAMINATION OF THE CHEST ORGANS.

Note: Former military personnel must have a medical characteristic and a copy of the extract from the discharge order.

Candidates for police service must provide documented results of the above examinations conducted within one month prior to the medical examination at the M(VL)K

LIST OF MEDICAL DOCUMENTS FOR PERSONS ENTERING MILITARY SERVICE UNDER THE CONTRACT TO THE UNITS OF THE NATIONAL GUARD OF UKRAINE

1. REFERRAL OF THE COMMANDER OF THE UNIT FOR MEDICAL EXAMINATION ACCORDING TO

FORM (ANNEX 14).

2. MILITARY ID OR REGISTRATION CERTIFICATE FOR MEN, PASSPORT FOR WOMEN.

3. OUTPATIENT CARD FROM THE PLACE OF RESIDENCE.

4. A DETAILED EXTRACT FROM THE MEDICAL RECORD OF THE MEDICAL INSTITUTION AT THE PLACE OF RESIDENCE, WORK, SERVICE, OR STUDY FOR THE LAST 5 YEARS (the extract must be certified by the doctor's signature and the seal of the medical institution).

5. CERTIFICATES OF PSYCHIATRIST AND NARCOLOGIST (at the place of registration).

6. CERTIFICATE OF MANDATORY PROPHylactic vaccinations (certified by the seal of the institution).

7. CERTIFICATE FROM THE TUBERCULOSIS DISPENSARY.

8. GENERAL AN. BLOOD, URINE, AN. BLOOD FOR GLUCOSE, AN. BLOOD ANALYSIS FOR RV, HEPATITIS B, C; HIV; BLOOD GROUP, RESULTS FACTOR, BIOCHEMICAL ANALYSIS OF BLOOD (total bilirubin, ALT, AST).

9. X-RAY (FLUOROGRAPHIC) EXAMINATION OF THE CHEST ORGANS.

Note: Former servicemen must have a medical characteristic, a copy of the conclusion of the M(VL)K, and a copy of the extract from the order of dismissal from service. Candidates for contract military service must provide documented results of the above examinations conducted within one month before the medical examination at the M(VL)K.

LIST OF MEDICAL DOCUMENTS FOR PERSONS ENTERING HIGHER EDUCATIONAL INSTITUTIONS OF THE NATIONAL POLICE OF UKRAINE

- Referral from the personnel department for medical examination in the form / Appendix 4 /.

- Registration certificate or military ID for men, passport for women, 1 photo 3x4.

- Outpatient card. Lyceum students must submit a medical characteristic.

- A detailed extract from the medical record of the medical institution at the place of residence, work, service, or study for the last 5 years. (The extract must be certified by the doctor's signature and the seal of the medical institution).

- Certificate (certificate) of psychiatrist and narcologist /from the place of registration/.

- Certificate of mandatory preventive vaccinations (form 063/o) in 2 copies.

- General analysis of blood, urine, blood glucose, and blood test for RV, hepatitis B, C, and HIV.

- X-ray / fluorographic / chest examination.

Candidates for training must provide documented results of these examinations carried out within one month before the medical examination at the M(VL)K

LIST OF MEDICAL DOCUMENTS FOR PERSONS ENTERING HIGHER EDUCATIONAL INSTITUTIONS OF THE NATIONAL GUARD OF UKRAINE

- Referral from the commander of the unit for a medical examination in the form / Appendix 14 /.

- Registration certificate or military ID for men, passport for women, 1 photo 3x4.
- Outpatient card. Lyceum students must submit a medical characteristic.

- A detailed extract from the medical record of the medical institution at the place of residence, work, service, or study for the last 5 years. (The extract must be signed by a doctor and stamped by the medical institution).

- Certificate (certificate) of psychiatrist and narcologist /from the place of registration/.

- Certificate of mandatory preventive vaccinations (form 063/o) in 2 copies.

- Complete blood count, urine test, blood glucose, blood test for RV, hepatitis B, C, HIV, blood group, Rh factor.

- X-ray (fluorographic) examination of the chest.

Candidates for training must provide documented results of these examinations carried out within one month before the medical examination at the M(VL)K.

The cost of a medical examination is for women - 300 UAH, and for men - 349 UAH.
