Requirements to application

   

When applying in writing, you must state in any form all the facts of corruption violations known to you. If available, evidence can be attached to the application to confirm the circumstances set out in it. The facts indicating the existence of a corruption offense may be documents.

The application must contain the last name, first name, patronymic, place of residence, contact information.

You can also apply orally (in person or by phone). However, it should be remembered that your appeal must be recorded with your name and address.

Message details are:

surname, first name, patronymic of the person who probably committed the offense, his position and place of work;

the text of the message containing information about the facts of committing a corruption or corruption-related offense, other violation of the law that may be;

surname, name, patronymic, address and signature of the person who sent the message (the author of the message).

Messages about possible facts of corruption or corruption-related offenses, other violations of this Law of Ukraine "On the Prevention of Corruption" can be carried out by an employee of the relevant body without attribution (anonymously).

Requirements for anonymous reports on possible facts of corruption or corruption-related offenses, other violations of this Law and the procedure for their consideration are determined by this Law.

Anonymous reporting of possible facts of corruption or corruption-related offenses, other violations of this Law shall be considered if the information provided in it concerns a specific person, contains factual data that can be verified.
